What the sequence documents
The existing teaching source describes a right L5–S1 far-lateral corridor for the exiting L5 root, with foraminotomy and discectomy. L5–S1 is one treated motion segment. Decompression and disc removal at that segment do not count as two levels. No fusion procedure is described in this source.
The operative views and the final on-screen caption describe an intraoperative assessment. Patient-reported pain or function, follow-up interval and a complete complication record are not available for this teaching sequence. The video does not establish a durable clinical result.

Reasoning in the source teaching article
The following explanations are reproduced from the existing operative concept. They are educational context; a separate patient-level decision record is unavailable.
Anatomic problem
A foraminal or foraminal-extraforaminal disc herniation at the right L5-S1 level compresses the exiting right L5 nerve root within and lateral to the foramen. This pattern is distinct from the common posterolateral herniation that affects the traversing root in the lateral recess, and it requires the surgical target to be defined as the exiting L5 root rather than the traversing S1 root.
Why a far-lateral corridor
The exiting L5 root and the compressive fragment lie at or beyond the lateral margin of the foramen, away from the central canal. A far-lateral working corridor reaches this zone directly over the L5-S1 junction, allowing the exiting root to be decompressed without entering the central canal or working through the standard interlaminar window aimed at the traversing root.
Technical considerations, limitations, and risks
Facet and pars preservation are weighed against the bone removal needed for adequate foraminal decompression, because excessive resection at L5-S1 can raise concern for segmental instability and shift the plan toward stabilization. Risks include exiting-root or dorsal root ganglion irritation, dural injury, incomplete decompression, recurrent herniation, and bleeding in the foraminal corridor; when foraminal collapse, instability, or disc-height loss dominate, a fusion strategy rather than isolated foraminotomy may be more appropriate.
